Jeremiah Press, Inc., 5840 Wind Drift Lane, Boca Raton, Fl. 33433 (hereafter referred to as the publisher) agrees to publish __(#)___ copies of _______Title_______________ (hereafter referred to as the work) for ________Author____________________ (hereafter called the author) based on the following terms:
1. The publisher agrees to provide the following services: typesetting, blue-line proof, color cover proof, ISBN number, Library of Congress catalogue registration, copyright registration in author's name, scanning code and final production.
2. The author agrees to payment in full of $______ in two payments to the publisher:
1/2 of total cost on acceptance of camera-ready file;
1/2 on author's approval of final blue line proof and cover proof.
3. It is understood that this cost does not include:
a. costs of shipment from the printer to the author's premises;
b. any extensive editing or rewriting;
c. advertising, promotion, publicity, marketing, sales, distribution, storage, or cover artwork of the work.
4. It is understood that reprints for future editions of this work, whether it be abridged or adapted, shall be at an adjusted cost reflecting increases in printing prices at the time of reprinting.
5. It is understood that it is the author's responsibility to obtain necessary copyright permissions for extraneous material, not the author's own, included in the work.
6. It is understood that the publisher holds no legal or financial responsibility or liability for the accuracy of the contents of the work.

1. The publisher agrees to provide the following services: conversion of file, editing, proofing, correcting, typesetting, cover design, blue-line proof, color cover proof, ISBN number, Library of Congress Catalogue registration, copyright registration in author's name, scanning code and final production.
2. The author agrees to payment in full of $______ in three payments to the publisher:
1/3 of total cost on acceptance of computer file;
1/3 of total cost on author's approval of final copy;
1/3 on author's approval of final blue line proof and cover proof.
